Additional Forms
In some cases, CSE and CAN will require additional forms in order to process your Activity Request Form. All the forms can be found on CSE's LUCommunity portal .
One special form is the Liability Waiver Form. If your event involves any physical activities, you are required to fill in the Liability Waiver Form and submit a printed copy to the CSE office at least 24 hours before the event. Link to that form can be found here: RSO Liability Form 2020.
